Patient Forms

Now you can fill out your patient history and registration form before coming to the office. No more clipboards in the waiting room! Before your first visit do the following:

ImportantStep 1—Check that you have the latest version of Acrobat Reader. You must Acrobat Reader Version 8 or above in order to fill out the form on your computer. If you do not have Version 8 or above, you will not be able to save the information you type on your computer, and the form will arrive blank. To get the latest version, download  Acrobat Reader 9 from the Adobe website.

Step 2—Based on your age or that of your child, fill out the form below and save it to your Desktop or My Documents folder.

Adult Form (18 years and over)

Adolescent Form (10 to 17 years)

Pediatric Form (newborn to 9 years)

Step 3— Email the completed form using my YouSendIt drop-box page. This drop box is secure and meets federal standards for privacy of medical information. Follow the instruction on the screen and just remember where you stored the completed form. You may choose to just fill out the form on your computer, print it, and bring it with you to your first visit. (Please note – Connecticut patients should only use the email method and should send the form several days before their visit)

If the form won’t load: open your new Adobe Reader program. Click on Edit, then select Preferences from the menu. Then select Internet from the menu. Under the Web Browser Options, un-check the box that says, “display PDF in browser”. This should resolve the problem.

Step 4— Read How To Report Symptoms in preparation for your office visit. Some patient find it useful to jot down a list of concerns to bring to their first visit.
